Assisted Reproductive Technology

Assisted reproductive technology (ART) is a treatment technique used to treat infertility. It includes fertility treatments that involve a woman’s egg and a man’s sperm. Here eggs are removed from a woman’s body. The eggs are then mixed with sperm to make embryos. The embryos are then put back in the woman’s body. In vitro fertilization (IVF) is the most common and most effective type of ART.

ART procedures sometimes use donor eggs, donor sperm, or previously frozen embryos. It may also involve a surrogate or gestational carrier. A surrogate is a woman who becomes pregnant with sperm from the male partner of the couple. A gestational carrier becomes pregnant with an egg from the female partner and the sperm from the male partner.

ART Procedure to begin IVF Cycle :

A woman looking for IVF treatment should call or visit within1 or 2 days of her menstrual cycle start. A doctor will guide her regarding a visit to the clinic for ART treatment. There she will get her first IVF shot for Antagonist cycle. In case of a down-regulated cycle, you can also visit the clinic on the 21st day of the period.

Medication for IVF

GnRH Agonist

Patient Monitoring

Blood Tests

During the monitoring process patients usually give 2-3 blood samples for estrogen and progesterone level in the blood. Estrogen allows measuring the relative egg’s maturity. The determination of maturity of eggs is between 100 to 200 units of estrogen. Moreover, with the progesterone level, doctors determine the ripeness of eggs.

Sonograms

It is a painless process, in which a radiologist inserts a small probe into the vagina to have a full image of ovaries and uterus on the screen, to count the follicles growing in ovaries, and to calculate the maturity of the endometrial lining of the uterus. Egg Maturity (Final Step)

When it is clear by blood test and sonograms that eggs are mature 18mm+, the final injection HCG is administrated to the patient between 9:30 PM to 11:00 PM on a specific date as per patient. Moreover, 36 hours recovery is scheduled after the HCG injection. For example—if a Monday monitoring of a patient shows a correlation of follicles at appropriate size and estrogen level maturity, she will be instructed to give HCG injection at 9:00 PM to 11:00 PM Monday evening.

Later the egg collection will be performed on a Wednesday morning. Doctors at KIC note the timing of the injection because its retrieval is critical as HCG may cause egg follicles to release prematurely. This can make the IVF retrieval impossible. When procedure begins—the patient is not allowed to take solid food after midnight before the procedure. Patients are not allowed to have sexual intercourse 2 days before the procedure to make sure the highest count of male sperm.
